- Eye Exam Dilated PRO ONLY (Must be on Pa Athletic Commission Form)
- Blood test results completed within the past 6 months for HIV, Hepatitis B-Surface Antigen and Hepatitis C http://requestatest.com/combative
- 38 & OVER MUST HAVE: Treadmill Stress Test
- 38 & OVER MUST HAVE: OPHTHALMIC EXAM
- 38 & OVER MUST HAVE: Neuro Exam
